Chapter 19:  there are two Example 10.8s.  Renumber the
second one to Example 10.9 and renumber Example 10.9 to 10.10.
No reference changes needed.

Chapter 2: reference after Example 14.5 to "Example 14.2" should
be to Example 14.4.  This should have been done in #7, but wasn't.

Chapter 19:  for both occurrences of "I am a friend of John and
you are a friend of James" read "I am a friend of you and John is
a friend of James" (i.e., swap "you" and "John" and adjust the
verb).
