The whole point of the new setup is that the index is maintained by
internal cross-references to particular points in the text, not to
page numbers.  Basically, somewhere in the text you have a marker
that says "here, put an index entry for broda", and that is
generated into a list of page numbers later on.

I wouldn't dispense with an index under any circumstances and, in
fact, getting the index to work in the new version took something
like a solid 2 weeks of basically all of my free time.

I know the answers to none of these for certain, but they over
perfect bound and cloth bound, and I believe the latter ==
hardcover, no?

If at all possible I would like to do so, yes; I believe this to be
easy with Lightning Source.
