I think using underlines would work, except in such cases where that isn't possible. For example, 'juliet.' for 'JUliet.'. Obviously, the ASCII convention for underlining when you can't: '_ju_liet.', looks horrid, but in that case using the canonical 'JUliet.' works in those (rarer and rarer) cases when underlining isn't supported.
