OK, I put it as an attachment to the Introductory Learning Material page.
I made the document as an Adobe InDesign native file before converting it to PDF. I no longer have the fonts Optima and Sabon and am in the process of replacing them in the original InDesign document. This will cause line breaks to re-flow and requires some time to reformat before I can continue with the rest of the book.
