first attempt at a proto-dictionary outline

Following is my first attempt at a dictionary outline, with estimated
page counts.  I'm including Nick in this, since he is doing lujvo, and place
structure paper, and he may have input on those sections (as well as
general comments on the rest).  

Not everything here may make it into the first edition, due to time and
space considerations, however, there may also be things missing, and I haven't
been padding the estimates, even though most things we produce exceed the
original page count estimates.  Thus the dictionary may end up being two
volumes in order to be perfect bound and hold together - in which case the
split is obvious between a reference book and a dictionary proper.  Certainly
by any second edition, when we have expanded lujvo definitions for all
lujvo, many more lujvo, and more English entries per lujvo, the dictionary p
propoer will have to be a separate book.
