Let me try again.

The idea is first to expand the lujvo into its components:
    vanjikru -> jikru be lo vanju
    kujmikce -> kurji gi'e mikce

Then the place structure of the lujvo is formed simply by all the
places of the components that are left unfilled:

 ko'a jikru lo vanju be ko'e  ->  ko'a vanjikru ko'e
 ko'a kurji ko'e gi'e mikce ko'e ko'i ko'o  -> ko'a kujmikce ko'e ko'i ko'o

In the second case, some of the places of the components
overlap: the x2 of kurji (the person taken care of) is the same
as the x2 of mikce (the patient). But there is no rule to force
this interpretation. {kujmikce} might also mean, for example,
{mikce be lo kurji}: a doctor for caregivers. I just picked the
one that seemed to me to be most likely. In other cases the
choice might be less obvious.

One possibility would be {sakmi'i}: {minji be le nu sakci}.

The place structure would be: {ko'a minji le nu ko'a sakci ko'e ko'i}
"x1 is a machine for sucking x2 from x3".
