Destroy it; that'll get caughtby the cmavo-list structure you
created.

The latter.  Although I might modify the exact indexterm for each;
little school/girls in the second case, for example.

Ideally, drop the "or" and let the markup handle it visually.  If
that doesn't seem possible, put in a FIXME: comment and I'll fix the
code not delete the "or" on the way out.
