' only occurs directly between vowels. A cmevla must always end in a consonant followed by a mandatory pause, which means that ' at the end of a cmevla would never have a vowel directly after it (to say nothing of the fact that most words begin with consonants). In other positions, ' is not considered distinct from other sounds.
[h] is a consonant sound (of the barest sort), but ' is not really treated as a consonant in Lojban. When you count the letters in, say, "ko'a", for practical purposes, there are only three: k o a. You might think of "o'a" as simply a sequence of vowels with a brief sound placed
between them to make them pronounceable.
