I like the sound of {a'o kamka'o}, but it doesn't seem like it could
catch on; too many syllables; I know of no language which has more
than 4, with many that can be said quickly in about 2).
Just {kamka'o} may catch on.
I almost feel like proposing that there be a vocative specifically for
this purpose.
