I'm going by the Book, which claims that a Lojbanist would see no reason 
why "ko'a" should not apply to "le karce" (this may seem odd, but 
remember that the Lojbanist in question may be a computer).  If you're 
likely to refer to your grandmother a lot, assign ko'a to her; otherwise 
just use normal anaphoric reference.
