As an orthographic omission, I think it could be omissible in all contexts except in: a'i, e'i, o'i, a'u, y'y, .i'V, .u'V, Vi'V, Vu'V.

The omission would have to be done from left to right though, so for example if you have "bi'i'a", you could omit the first apostrophe and write "bii'a" but now you can't omit the second one, or you could leave the first one in, omit the second one and write "bi'ia", but you can't now go back and omit the first one even though it looks like an omissible context, because "biia" is "bi,ia" without any apostrophes.
