Assuming both indexes had the same content, which do you prefer for
a *PRINTED* book, and how much?

How much difference does it make if the average length of a section
is, say, 3 pages?

If you could make it do

lo                §1.1¶1-4 , §2.3¶5 , §3.4¶10 , §7.8¶2
klama          §1.1¶1 , §18.12¶4
abstractions §2.3¶2,3 , §5.7¶8 , §12.18¶8

But you couldn't make it do page numbers, I'd be okay with it.
