I was thinking of tossing it out entirely, actually; I don't find it
an interesting addition given the thoroughness of the index.

All of it. Anything that has to do with how the XML gets turned
into a PDF, really, which is all about LaTeX; about generating the
right LaTeX from the XML and tweaking the LaTeX with extra .sty
files or whatever.

I want it to be awesome. I want to be proud to publish it. So: all
of the above. I'm perfectly happy to have tags in the XML that are
only used by the LaTeX/PDF if necessary/helpful (and vice versa for
the HTML, although CSS makes that largely unnecessary).
I know where all the hooks are; I can make the LaTeX look like
pretty much anything you want. What I don't know is what the LaTeX
should look like to get particular visual results.
