It can fail if the prefix is CVC or non-existent, but always works with
4-letter prefixes.  The only constraint then is "starts with a consonant,
ends with a vowel".

I think it always works with a CVC prefix too. Type-3 always have a consonantal syllable, so they can never be confused with lujvo, which never have consonantal syllables. So yes, it's type-4s where it sometimes fails, because the apostrophe may create either a lujvo or a slinku'i.
